Dick Gin, A.T.C., a native of San Luis Obispo, graduated from California State University, Fresno and M.E.D. from the University of San Francisco. In addition to his work in the clinic, Dick provides athletic training services to high schools around the county. He has served as head athletic trainer for the San Francisco Giants, Class A baseball team and often lectures on the care and prevention of athletic injuries. Dick is a certified member of the National Athletic Trainers Association and the California Athletic Trainers Association.

 
    Don Olson, P.T. graduated in 1978 from the Physical Therapy Program at California State University, Long Beach. His experience as a four-time All-Conference baseball player who survived rotator cuff injury motivated his interest in sports medicine rehab. Don has expertise in extremity rehabilitation, with special interests in the throwing shoulder and golf related injuries. (Don is a single digit handicap golfer himself.) He is a member of the Private Practice, Sports Orthopedic and Aquatic Sections of the American Physical Therapy Association.

T.C. Chaffey, B. S., M. S., is a Cal Poly graduate living in San Luis Obispo county since 1978. T.C. has been with Creekside Physical therapy for the past five years. She heads up the aquatic exercise program utilizing her extensive teaching, coaching and work experience in the aquatic environment. T.C. was a competitive masters swimmer, triathlete and marathon runner until a traumatic fall resulted in a multi-level cervical fusion surgery. Karen Empey - Creekside PT
  Karen Empey P.T., is a Cal Poly alumnus in Biological Science and graduated from Daemen College, Buffalo, New York in 1994 with a degree in Physical Therapy and a minor in Biology. She has practiced in San Luis Obispo County since 1994 in a variety of settings including acute care to outpatient orthopedic practice. She enjoys post-operative rehabilitation as well as geriatric patient care. She has played competitive sports including soccer and volleyball and has a passion for fitness as well as health and nutrition. Karen is a member of the American Physical Therapy Association.

Eric Nelson, P.T. is a 1976 graduate of Cal Poly and a 1980 graduate of Mayo Clinic school of Physical therapy. He has been practicing orthopedic physical therapist in San Luis Obispo since 1982. He is also a long time member of the American Physical Therapy Association , orthopedic and sports section. Special interests include bicycle racing and touring, recreational trail running and surfing. He is a member of the SLO bike club, SLO Nexus cycling club, SLO bike coalition and the CCCMB. He brings a wealth of experiences to our clinic and happy to be a member of our clinic.

Kathleen Milner PT, Creekside Physical Therapy

 

Kathleen Milner, P.T., a Cal Poly alumnus and graduate of University of California, San Francisco with a Masters in Physical Therapy, 1993. She has practiced in Santa Barbara and San Luis Obispo Counties since 1996, with experience in acute care, home health, and outpatient orthopedics. She has specialized training in breast cancer rehabilitation and breast cancer related lymphedema. Kathleen
played softball for Cal Poly and she enjoys running and swimming.

 

 

 

Jackie Cabalka, P.T. graduated from Cal Poly, San Luis Obispo, with a Bachelor's degree in physical
education and then earned her Master's in physical therapy from UC San Francisco. Jackie earned her Doctorate in physical therapy from Creighton University in Nebraska. She also received a certification in pilates for rehabilitation after completing a year-long training series through Polestar education. Jackie has been utilizing modified pilates in her physical therapy treatments for a wide variety of orthopedic and neurological conditions since 2001. She has developed treatment programs for low back pain, osteoporosis and breast cancer rehabilitation among many others. She also teaches pilates privately. A native to the central coast, Jackie has 3 children and stays active through pilates, running, cycling and hiking. She is a member of the American Physical Therapy Association and is very pleased to be joining the team at Creekside physical therapy.

  Jackie Cabalka PT, Creekside Physical Therapy   shelly   Shelly Chisamore started with Creekside in 1997. She is responsible for managing our front office proceedings which includes scheduling, billing questions, and insurance authorizations. Shelly brings along with happy, friendly face, a diverse insurance knowledge and is available to help you with any questions that may arise during your treatment.

Laura Maikowski comes to us from Vermont to fill our front office needs. She brings to us 15 years of front office medical experience and a warm and caring personality. Insurance authorizations, scheduling and management are only a few of her responsibilities. Her dynamic and sparkly personality, greet patients with a smile and always a positive outlook. Laura’s interest include gourmet cooking and baking. She moved to the Central Coast with her fiancée in 2004 and is a welcome addition to our front office.
